Digital care transition for hospitals

Bundle care needs and the responses of participating care providers. In a presentation we clarify the right access. Pflege Patienten replaces neither your discharge management nor the HIS.

  • Posting care needs is free of charge
  • Runs in the browser, no HIS interface required

Hospitals are activated separately. Immediate self-registration for hospitals is not promised at present.

One process, three perspectives

Fictional example

  1. Describe the need, bundle the responses

Describe the care need and the open services. Handle the responses of participating providers to the request in one place.

    Type of care
    Short-term care
    Place
    00000 Sampletown
    Planned discharge
    In five days
    Open services
    Personal care, medical treatment care
  2. Does the request suit the location?

Check whether the requested care or individual services suit the own location.

    Sample care home
    Interested in the short-term care
    Sample care service
    Question about the treatment care
  3. What is on record for the request?

See which responses there are to the request. You download the record of the search as a PDF.

What we clarify with your hospital

In a joint introduction we show the process with invented sample cases. No installation is needed.

  • Which types of care and regions matter to your hospital
  • Who posts care needs and handles responses internally
  • Which requirements apply to use, data protection and activation
  • Which product functions are available today and which are not

How a case works

Post the care need

Type of care, place, start and services – pseudonymised, without name or address.

Providers are informed

Matching registered home care services and care providers in the region receive a notice.

Select and confirm

You select a provider together with the patient; both sides confirm the acceptance.

Software for follow-up care – your discharge management stays

Pflege Patienten supports the placement step of follow-up care. You see your cases together with their status.

  • Care request, responses and coordination at the case
  • Treatment, documentation and handovers remain in your discharge management
  • Record of the search for care as a PDF

A connection to your HIS is not available at present; Pflege Patienten runs in the browser.

Team and data protection

Every person works with their own access; for hospitals the second factor at sign-in is mandatory. Pflege Patienten processes the case data as a processor. You pass on name and address directly to the provider only after the selection.

Data security and data protection Data processing agreement

Posting care needs is free of charge.

The paid provider function concerns expressing interest in other people’s requests. There is no platform fee for posting clinical care needs.

Access for your hospital

Hospitals can only post cases once the operator has activated access for hospitals. Until then the portal shows the notice “Pflege Patienten is not yet available for hospitals.”

In a presentation we tell you the current status for your hospital and show the process with invented sample cases. No installation is needed.

Medical practices, advisory services and care support centres register directly and post cases after the check.

Questions from hospitals

Does a hospital have to replace its systems or set up an interface to its hospital information system (HIS)?

No. Pflege Patienten is intended for the referral step and runs in the browser. It does not replace a hospital information system, and no interface is required. Further integrations are not currently available.

Can several team members work with one account?

Yes, with personal access: each person signs in with their own access. You manage your team with roles and areas and set up deputies. Sign-in with a second factor is mandatory for hospitals. Please do not use shared sign-in details.

Who sees the information about a case?

That depends on the role, the permissions and the status of the agreement. Matching care providers first see the details for the referral – including type of care, place and approximate distance, services with times, care level (Pflegegrad), age group, diagnoses, short description and the referring organisation with its work contact – but no names and no address. Only the home care service or care provider that is selected receives the telephone number and email address of private individuals. We cannot promise that nobody could ever identify the person concerned; details are given on the page Data protection and security.

What happens if no care provider responds?

In that case there is no response and no acceptance yet. You can see the status of the responses to your care need at any time. If no acceptance has been agreed for any service after 14 days, the care need is closed as ‘not placed’; you receive an advance warning on day 11. The platform does not replace the further organisation of care.

Let us clarify access for your hospital.

Briefly describe your hospital and the intended use. Please do not send any patient data through the contact form.
