Place patients: organise follow-up care

Post the care need once. Participating care providers that match can respond with interest, questions or a refusal.

  • Posting a care need is free of charge
  • Practices and advisory services register directly
  • Hospitals after separate activation

Fictional example

  1. What needs to be organised?

Place, start and the services needed are the starting point. Only post details that you may pass on in the permitted process.

    Services
Personal care in the morning, help in the household
  2. Who has responded?

Assign interest, questions and refusals to the request. A response can also cover individual services.

    Help in the household

  3. What happens next?

Clarify with the provider whether and how the care can be taken on. An expression of interest is not yet a commitment to provide care.

Pass on a care need from your practice or advisory service

When a patient needs support, you post the care need and handle the responses of participating care providers in one place. Registration is checked; posting is free of charge.

Everyday care is no longer enough

Describe the help needed and the desired start. Matching participating home care services can respond to individual services or to the whole care.

For the time after treatment

Note what has to be organised by when – for example care at home or a time-limited short-term care stay.

You are supporting a family

You help a family find their way. Agree which request you may accompany within the permitted scope; the family can also post a request privately itself.

Register as an organisation

From registration to the request

Create the organisation

State the practice or advisory service you work for.

Wait for the check

The operator checks the access. Every person uses their own account.

Post the care need

Describe the care need you may pass on and handle the responses – without the name and address of the person concerned.

Looking privately or working in a hospital?

Are you looking for care for yourself or a relative? Use the care request assistant for private requests.

Do you work in a hospital? Hospital access is clarified separately; a presentation is the first step.

Fewer calls, one overview

  • Pass on the care need once instead of calling every provider
  • See interest, questions and refusals at the case, also for individual services
  • Agree the matching care and download the record of the search as a PDF

If your home care service can no longer cover one area, you post the open part free of charge – with the same access.

Posting a care need stays free of charge.

Hospitals, practices, advisory services and care providers pay no platform fee for posting a care need. The paid location subscription concerns providers that express interest in other people’s requests.

The platform replaces neither the consent check nor the individual agreement of the care. Acceptance is not guaranteed.

Questions about placement

Who can post a care need?

Care needs can be posted by medical practices, advisory services and care support centres, care providers passing on a care need, and private individuals and their authorised representatives. Hospitals and their social services can post cases once the operator has activated access for hospitals. Registration takes place in the portal – as an organisation or as a private individual; the page ‘Registration’ describes the steps.

How much does posting cost?

There is no platform fee for posting a care need. This says nothing about how the care services themselves are funded – that depends on your long-term care insurance fund (Pflegekasse) or statutory health insurance fund, or on your agreement with the care provider.

What happens if no care provider responds?

In that case there is no response and no acceptance yet. You can see the status of the responses to your care need at any time. If no acceptance has been agreed for any service after 14 days, the care need is closed as ‘not placed’; you receive an advance warning on day 11. The platform does not replace the further organisation of care.

Can I send patient information through the contact form?

No. The contact form is only intended for general questions, requests for a presentation and data protection requests. Please do not send any diagnoses, prescriptions or other patient details there. Care needs are posted exclusively in the protected portal.

Organise support without starting every contact on your own.

Practices and advisory services register their organisation in the portal. Hospitals first request a presentation.
