Find a place in a care home
Are you looking for permanent care and support in a facility? Describe which care is needed and where you are looking. Matching participating facilities can respond to the request.
A free place or admission cannot be guaranteed.
Find a care home place: when a care home fits
The decision is often made after a fall or a hospital stay: care is needed around the clock, relatives can no longer provide it, or the home is no longer safe.
Whether moving is the right step is decided by the person in need of care or their authorised representative. Care counselling, for example at a care support centre, helps to weigh it up.
Living there permanently or bridging a period?
The decision should depend on the individual situation. The platform does not make a professional assessment.
Living in a facility for the long term
Looking for a care home place is about long-term care and living in a facility. Besides the care needed, personal wishes and everyday life play a part.
Only for a limited period
If an inpatient solution is only needed for a limited period, looking for short-term care may be the better task.
What matters to you about the facility?
Your personal priorities help in conversations with facilities. The list replaces neither a visit nor individual advice.

How the search for a care home place works
Preferred region and start
You state the postcode, start, care level (if known) and the help needed in everyday life.
Care homes respond
You see interest, questions or refusals at your request. You arrange visits directly with the home.
Agree the place
The place is agreed once both sides have confirmed. You conclude the residential contract with the care home.
What the platform costs
Registration and the care request are free of charge for private individuals and relatives. The cost of the later stay and possible benefits from funding bodies are discussed separately. The request does not mean any funding has been agreed.
An expression of interest is not yet a binding commitment. Questions about suitability, actual availability, costs and admission conditions remain to be clarified in direct conversation.
Are you looking on behalf of a relative?
Involve the person in wishes and decisions as far as possible. Clarify who may accompany the request and pass on details.
Perhaps a move is not yet necessary: a home care service or day care relieves everyday life. For only a few weeks, short-term care fits. Special forms: out-of-hospital intensive care and assisted living.
Questions about a care home place
Which type of care do I choose if I am unsure?
Choose the type that comes closest to the situation and describe the care need in your own words. A request covers exactly one type of care; care providers only receive a request if their offer fits this type. You can change the type of care when editing – services that do not fit the new type are dropped. A care advice service, for example at a care support centre or your long-term care insurance fund, can help you choose.
Who sees my request?
Registered home care services and care providers whose service area covers the place you entered and whose offer fits the request. They see the details needed for matching – such as the type of care, place and approximate distance, start, services required and care level – but no name, no address and no date of birth. A care provider only sees your telephone number and email address once you have selected it.
What happens if no care provider in my region takes part yet?
Pflege Patienten is organised regionally. If no matching home care services or care providers are registered for the place you entered yet, nobody is notified of your request and you will not receive any responses. In that case, contact care providers near you directly at the same time, or a local care advice service, for example a care support centre or your long-term care insurance fund. The page 'Regional participation' shows the current status. Whether participating providers can be reached at your place is shown by the care request assistant, before you create an account.
What does a request cost private individuals?
Posting is free of charge – registering and posting a request cost private individuals nothing at Pflege Patienten. This only concerns the platform: what the care itself costs and which services your long-term care insurance fund (Pflegekasse) or health insurance fund covers is something you clarify with the care provider and with your fund.
Request a care home place
Free of charge for private individuals. The care request assistant first shows whether participating care homes can be reached at the place.
