Refer care onwards – in full or for individual services
Existing care no longer fits your capacity, rounds or services? Post the open need free of charge and look for a suitable follow-up solution with participating providers.
An existing provider access can be used for this too; if needed, the way leads to the login first.

Typical reasons for follow-up care
If a person in need of care has to change their home care service in full or in part, follow-up care organised early helps everyone involved – for example, when:
The handover has to be agreed carefully with the people and organisations involved. A published request does not transfer responsibility and is not yet agreed follow-up care.
How to proceed
Describe the open need
In the portal you choose “Post a care need”: which support is needed in which region and from when – pseudonymised, without name and address.
Sort the responses
Matching participating providers whose service area covers the place can express interest, ask questions or decline.
Agree on the handover
Clarify directly with those involved whether and when another provider takes over. It is only agreed once both sides have confirmed.
Data protection in brief
You post the case pseudonymised – without the name, address, date of birth or insurance number of the person concerned. When posting, you confirm that their consent has been given.
For organisations that post cases, Pflege Patienten processes the case data as a processor; the basis is a data processing agreement with your organisation.

Posting and taking on cost different things
Posting is free of charge: there is no platform fee for your own open needs – also for home care services and care facilities and also without a location subscription. Use your existing account; you do not need a second one.
The location subscription (€99 plus VAT per location per month) only concerns taking on care – that is, the other action: expressing interest in a request from others. It starts with the first expression of interest after you explicitly confirm the price and contract terms and can be cancelled at the end of the current contract month. A successful placement is not a condition for it.

Only household help to refer onwards?
There is a separate page for help in the household. The background is the draft Long-Term Care Reorganisation Act (PNOG) – a draft, not yet passed.

No access yet?
Use the matching way for organisations. Everyday support providers use their own access. Existing users can log in directly.
We do not promise a response or an acceptance. Keep other suitable ways of searching and coordinating in view.
Questions about referring care onwards
Can I refer just a single service onwards?
Yes. You post only the service category you are looking for a provider for – for example household help or medical treatment care. You state services that are already covered and those still open separately in the care need.
How quickly will there be responses?
We cannot promise that. Whether and when providers respond depends on who takes part in the region and has capacity. Registered providers whose service area fits are informed once the care need is published; you see the responses in the portal on your care need.
Do I need a location subscription for this?
No. Registration, the entry in the provider directory and posting a care need are free of charge. The location subscription only concerns taking on care – that is, when your location itself expresses interest in requests from others.
What details do I give about the person concerned?
Only what is needed for the referral, among other things the type of care, postcode and town, start, the services needed, care level, age group and diagnoses – pseudonymised, without name, address, date of birth or insurance number. Before posting, you confirm that the consent of the person concerned has been given.
